Full Service Hospice Care

Established in 2001, Unity Hospice Care is a full-service Hospice care provider. We are committed to providing compassionate care to our patients and families. We believe that each individual should be granted the opportunity to die in a dignified and comfortable manner. Our focus is to enable our patients to live as fully and comfortably as possible while providing spiritual and palliative care through God’s grace.

This special care is primarily provided in the patient’s home. However, those living in nursing homes, assisted living facilities or other in-patient facilities may also be eligible.

ELITE HOSPICE HONORS

Unity Hospice Care is proud to designated as ELITE HOSPICE by HEALTHCAREfirst (2019, 2020, 2021, 2022 and 2023). Hospice Honors is a landmark compilation of hospices providing the best patient and caregiver experience. This prestigious annual review recognizes hospices that continuously provide the highest level of quality as measured from the caregiver’s point of view. Award criteria were based on Hospice CAHPS survey results for an evaluation period of October 2017 through September 2018.

In order to receive the award, hospices must be currently working with HEATLHCAREfirst and must have had at least one complete survey returned in each quarter of the evaluation period. Award recipients were identified by evaluating performance on a set of twenty-four quality indicator measures. Individual hospice performance scores were aggregated from all surveys with a final survey status of complete for the evaluation period and were compared on a question-by-question basis to a national performance score calculated from all hospices contained in the HEALTHCAREfirst Hospice CAHPS database. Hospice Honors recipients include those hospices scoring above the HEALTHCAREfirst Performance Score on twenty of the evaluated questions.
